Precision Medicine Moves to the Forefront
Advanced gastric cancer remains associated with poor survival, particularly because many patients present with unresectable or metastatic disease. The newly published expert consensus highlights how precision medicine is replacing the historical one size fits all treatment approach by recommending routine biomarker testing before selecting first line therapy. The recommendations emphasize that identifying molecular characteristics can improve treatment selection while helping clinicians better match patients with therapies most likely to provide meaningful benefit.
The consensus outlines 27 evidence based recommendations covering biomarker assessment and treatment selection. Routine testing for mismatch repair status, microsatellite instability, HER2, PD L1, Claudin 18.2, Epstein Barr virus, and selected emerging biomarkers is recommended to guide individualized therapeutic strategies whenever clinically appropriate.
Biomarker Testing Drives Treatment Decisions
The recommendations place particular emphasis on integrating biomarker testing into routine clinical practice before initiating treatment. Patients with MSI high or deficient mismatch repair tumors may derive substantial benefit from immune checkpoint inhibitor based therapy. HER2 expression continues to guide anti HER2 treatment, while PD L1 expression remains important for determining which patients are most likely to benefit from immunotherapy combinations. Claudin 18.2 expression is also identified as an increasingly important therapeutic target, particularly for patients whose tumors demonstrate high expression.
The document also recommends a more detailed four category classification of HER2 expression rather than the traditional positive or negative approach, reflecting the expanding role of targeted therapies in advanced gastric cancer management.
