Heading into the closing minutes, Senegal found themselves 2-0 up after a sensational display against a sluggish Belgian outfit.
Habib Diarra and Ismaïla Sarr both struck either side of half-time, setting up a commanding lead.
Romelu Lukaku, on as a half-time substitute, pulled one back in the 86th minute before skipper Youri Tielemans equalised with an 89th minute header.
In extra-time, Senegal looked to run out of steam. Belgium dominated possession but offered little going forward.
Until the 120th minute, Tielemans challenging for a cross alongside Lamine Camara. Despite the appeals, the referee did not give a penalty in real time.
After being asked to check the monitor by the VAR, the officials decided that Camara made contact with Tielemans as they both slid in.
Tielemans took the spot-kick to send Belgium into the round of 16.
Senegal were incensed, surrounding the pitchside monitor before doing their best to delay and distract once the spot-kick was given.
One player lay right on the penalty spot. All told, the time from the review being initiated to Tielemans taking the penalty was seven minutes.
After the stunning comeback, Belgium will play the winner of the USA vs Bosnia and Herzegovina which will kick off at 2am UK time (9pm ET).
